Accommodations must be formally requested by the student in writing. All requests should be returned to [email protected]. Students are encouraged to provide the College with prompt notice of their needs to allow time for an interactive process to take place between the student and Provo College. Please contact the Campus ADA Coordinator with any questions.
To request an accommodation, a student and their healthcare provider must complete and submit the ADA Participation and Accommodation Request Form (link provided on this page), including supporting documentation that should be current (no more than one year old) and must include the following information:
- A diagnosis of the disability and any accompanying testing results;
- A detailed description of the specific impairment, limitations, functional need, and the medical justification for such need;
- A recommendation for the type and duration of the accommodation(s) needed; and
- The professional credentials, contact information, and signature of the health care provider.
Accommodations are provided through an interactive process, which is meant to be informal and is aimed at identifying the precise limitations resulting from the disability and potential reasonable accommodations that could overcome those limitations. Depending on the case and the documentation presented, an evaluation by a health professional designated by Provo College may be necessary.
Upon receiving and reviewing the student’s supporting documentation, the interactive process may continue, or a decision may be made on the request. Provo College may on its own obtain a professional determination of whether requested auxiliary aids are necessary and, if so, what kind of aids would be most appropriate. Provo College may also require an independent medical examination by a healthcare provider of its choosing to assist it in determining its response to a request for accommodation.
Note: Documentation regarding an Individualized Education Program (IEP) will not be accepted as sufficient documentation to support an accommodation. An applicant or student with an IEP must also submit the ADA Participation and Accommodation Request Form completed by their health care provider.
